Most airline accidents are attributable to flight crew error. In this book the authors go beyond investigating accidents: they ask why any highly experienced crew might have been vulnerable to error. Here the authors examine 19 major accidents in US air carrier operations (1991–2000). They give a brief description of the accident and the National Transport Safety Board conclusions about the causes of the accident and contributing factors. The significant events leading up to the accident are examined, leading to thoughts on why the crew were vulnerable to errors. Each chapter ends with a short summary and a discussion of the implications of the findings. |
