In April 2002 an accident involving an industrial radiography source containing 192Ir occurred at Cochabamba, Bolivia. A faulty radiography source container had been sent by bus from Cochabamba back to the capital La Paz, 400 km away. This gave rise to the potential for serious exposure to the radiographer, the passengers and bus employees. This is the report of the IAEA team that were sent to investigate the accident. It gives background information, the chronology of the accident, follow-up actions, the IAEA response and the findings, conclusions and recommendations. |
