This book aims to show that something can always be learned from disasters and failures. It describes disasters that occurred in a wide range of industries: the Hindenberg; UK railway disasters; the Uskmouth turbine failure; Challenger; Columbia; Ro/Ro ferries; bridges; the de Havilland Comet; and Chernobyl. Each of these descriptions is accompanied by ‘Comments’ and ‘Lessons learned’. The second Part of the book includes words of wisdom and views from experts that provide background to the failures already discussed. The last Part looks for conclusions and common threads. |
