This book looks at and analyses the headline accidents involving man’s use of technology, and examines the causes. The authors’ observe that the rate of error education (learning) is in direct proportion to the number of errors being made. They explain how we can analyse and learn from our mistakes and how human weakness in harnessing technology is behind many headline-making accidents that have occurred in the last 200 years. Incidents such as the Concord air crash in 2000, the sinking of the Titanic, car and truck crashes, and railway accidents (from the Rocket to red lights at danger) are discussed in detail. |
